The fourth round of the Macedonian First Football League (PMFL) ended with two derby matches in Skopje: Vardar beat Sileks 4:1 at Arena "Todor Proeski", while Shkëndija defeated Struga TL 2:1 at the Football Federation of North Macedonia (FFM) ground.

Shkëndija remain the only team with a perfect record after four games. The Tetovo side now have 12 points following the 2:1 win over Struga TL.

Besart Ibraimi provided the decisive moment in that match, converting a penalty in the 85th minute; it was his first goal of the season and the 229th of his career in the PMFL.

Sileks and Struga TL suffered their first defeats of the campaign and, together with Tikvesh, sit on seven points, sharing the chase for the top spots.

Bregalnica, Vardar and Arsimi each have six points — meaning Shkëndija keep a six-point cushion over the reigning champions, Vardar. Shkëndija Arachinovo follow with four points, Skopje have two (two draws), and Bashkimi remain bottom with zero points and a goal difference of 0:15.
