FZO removes Myfortic copays — transplant patients to collect therapy free from August 26, 2026
Health Insurance Fund removes copays for Myfortic after generics proved temporarily unavailable; savings of about 1 million denars expected.
Patients with transplanted organs will be able to collect the drug Myfortic (180 mg and 360 mg) from pharmacies without any copay starting tomorrow, after the Health Insurance Fund (FZO) reversed a recent copay decision. The move ends a two-month episode that had raised alarm among transplant recipients.
Two months ago the FZO changed the reference prices on the Positive List of medicines after cheaper generic products — such as Trixin — entered the market. That decision suddenly produced a heavy copay for the original product Myfortic 360, which patients must take daily and lifelong to avoid organ rejection, creating an initial copay of 3,450 denars (later corrected to 1,439 denars per box).
Why the problem arose
The Health Insurance Fund says that, at the initiative of director Zlatko Perinski and in coordination with the Ministry of Health, it completed the procedures to remove the copays. A key reason for the rapid reversal was information that the alternative generic medicines meant to be free are currently unavailable on the Macedonian market due to production-technical problems. Rather than shift that burden onto patients, the Fund restored the original product to the fully covered medicines category.
Specifically, the copay for Myfortic 180 mg (previously 615 denars) and for Myfortic 360 mg (previously 1,439 denars) has been removed. The Fund also renegotiated the purchase price for the medicine, securing a further reduction that is expected to save roughly one million denars for the second half of the year.
The decision responds to criticism from the Association of Kidney Patients “Nefron”, which warned that over 200 kidney transplant patients were put in a difficult position because experimenting with treatment or switching a proven drug for a generic alternative poses life risks for them.
The change was announced on 25 August 2026 at 14:11; the Fund says patients will be able to pick up Myfortic without payment from 26 August 2026.
